ADVANCE CONSENT AGREEMENT

I agree to have the HMR products I have selected automatically shipped to me at the time intervals I have indicated.

I authorize Hackley Health Management to bill my credit card for each Express Delivery Club order at the time of each shipment.

I understand that I will receive an email from Hackley Health Management Five days before the item(s) is/are shipped. If I want different products or do not want this order shipped at all I must notify Hackley Health Management by phone at 1800 521-9054 prior to the shipping date.

I understand that HMR products are considered food products and are non-returnable and non-refundable.

I understand that this agreement will be ongoing until I notify Hackley Health Management by phone at 1800 521-9054, that I wish to discontinue participation in the Express Delivery Club.

Due to the fact that these items are considered food products, they are non-returnable and non-refundable.
